The relatively small change in outstanding from 6/30 to 9/30 doesn't appear to have come from a new issuance. Apparently (I don't know for sure) when restricted shares are issued as additional compensation they aren't considered to be outstanding until they vest. The exact difference (572,263) appears under Restricted Stock Awards Outstanding as "Total shares vested" and that seems a very unlikely coincidence. The vast majority of the shares were issued in previous periods to AB and JH with a small amount going to other personnel.
