Needless to say, I think it happens quite a lot even in states where it isn't officially legal, and always has - I agree, it's just not talked about.
I volunteer in my local hospital and docs know when it's time, they have to work with the patient's family to prevent law suits.
Also, I am trained for my hospital's No One Dies Alone* vigils - I had one experience where the man was in ICU on tubes to keep him alive, the family didn't want to let him go yet they couldn't handle visiting him often, so the NDA team jumped in.

I feel strongly we are not born alone and no one should die alone.
*No One Dies Alone started in Eugene, Oregon in 2001, and now volunteers are in hospitals across the country.
