How does one get to be appointed as a general, and by whom? Whom does a general report to, and who reports to a general?

generals are “appointed” by a military promotion board made up of general officers of the branch in which the officer in question serves, which board recommends (to congress) the promotion from Colonel to Brigadier General (the lowest of five levels of general officer in the US military). Congress then approves or dis-approves the promotion and the President signs the Promotion Order.
