It's called a Form 211, and is, as you say, to be filed only by a sponsoring MM. Said MM is essentially vouching for the accuracy of the company's statements and financial filings. No non reporting company has ever returned to quoted trading that I know of. Neither this or PDXP will break that mold.
It certainly appears that Patrick Johnson is unaware of the rule.
