i would suggest speaking directly to your broker for requirements. if you do not have a certificate, then you cannot have a certifcate#. TDAmeritrade's rep called me after i mailed completed paperwork and informed me there would be no charge and that USMJ's PURA dividend is a special situation and that there would be no charge and my shares could now be freely traded.
