I think the gross margin tells the story: sequential improvement quarter-over-quarter. If these sort of promotions have been happening, they are getting fewer and far between.
Various. Big name supermarkets across the US. May be store or region specific but I have seen this in many states. Some of the many locations I happen to have been to since I've first looked and inquired have run out of them. Likely some of these places have run sales to get rid of them as they haven't been restocked. I haven't looked into all of them but noticed some were advertised sales. Kroger has done sales and b1g1 coupon through their app but others seemed to be marked down by the particular store like a clearance item and not included in the circular.
