I haven't read this paper yet but thought it worth mentioning that the placebo effect, at least for some indications, is not necessarily confined to "positive expectations", i.e. concealing the fact a pt is taking a placebo, which has ethical implications when it comes to taking advantage of a placebo in practice since it involves deception. e.g.:
