or I suppose they used the statistical analysis, upon which they'll expound in the paper, to try and distinguish them....

Agree -if the diet/exercise NASH improvement effect is inverse to the NASH score then it makes sense to exclude milder cases. I would guess that the more obese one is the more difficult it would be to stick to a diet/exercise regimen... assuming obesity and NASH are directly correlated.
