These days mostly there are electronic exchanges, the most prominent being ARCA. There are a few actual "Market Makers" but anybody could be routing orders through them. Yes, way too much attention is paid to which one it is, because it doesn't matter at all. What matters, as always, are the objectives of the buyer/seller using the exchange.
