One advantage of the later stage patients is they do indeed have AD. The earlier stage patients are likely a mix of various dementias and frail elderly patients in overall decline.

Ran across a company recently that is running a new Phase III in APOE4 patients only based on a subgroup analysis of an earlier failed trial.

they are probably looking for a symptomatic effect, where it is easier to see improvement when patients have a lot of symptoms.

earlier stage is important for determining any impact on disease modification.


there is no such thing as a possibility for reversal. Once neurons die, they're not coming back.
