spockster, we have pictures of the packet sold on the shelves by CC. The price is clearly marked at $16.99 and covers the buyers 1st month of the subscription. In the packet there is a code that the buyer enters when they get home and on-line. The original contract that was filed between CC and GNF specified the packet that was to be sold. There is no free trial and only the 3-pack plan is sold with the packet. Other plans through CC are available on-line.

That being said the new contract reads that CC will keep their portion of the in-store sale and send the remainder to GNF. Which must mean they are keeping less than the $16.99 charged at the cash register.
