Because the criminals running scams like ITNS don't bother with fripperies like Form 4s, just as their promoters don't bother with the legal requirements of the Securities Act.

A cursory glance at any of the recent SEC cases against penny stock scams like ITNS shows both of these facts to be true.

And anyone who doesn't think that the fact that the "Emesyl" website is a straight cut and paste from the Zicam website is highly symptomatic of a pump and dump is in for a rude surprise.
