Well I'd assume Qualcomm for one at least were busy at work long before now at that, you might notice that a modem was one of the first things produced on TSMC's 20nm process.
Actually the simulation models are getting very good now, one can simulate the process of manufacturing and the 3D structure of the resulting circuit and get a good idea of how it will work, and tweak for manufacturability despite slight mask misalignments for instance. One needs to produce actual chips to check the correspondence with reality but simulations can do most of the real work from quite early in the design cycle.
