I would expect that anyone refusing to take the blood test would simply be denied coverage, or automatically placed in a high-risk category where the premiums would be high.

Whenever I've purchased term life insurance in the past, I was required to undergo a physical exam which included blood work and drug screening. I don't know if those are required for every policy, but they were required for the preferred rate policy I was purchasing. They were conducting due diligence to ensure I wasn't lying them them about my lifestyle and current health conditions on the application.

Lying to an insurance company on an application is considered fraud and can land you in jail if they find evidence you had knowledge of an existing condition and lied about it on the application. While they may elect not to file criminal charges against you, they would certainly deny any and all claims made against the policy. Insurance companies aren't stupid. They know if a person is refusing to take a test, there is a reason and it's likely going to cost them a lot of money.
