I agree with your points but you are missing the nuances of what happens behind the scenes on the hill via lobbyists and pressures on the AMA etc., and other healthcare folks in physician practices. Big pharma yields exceptional power. As we speak their are innumerable drugs that should be rx over others based on safety, efficacy, cost etc., and our not due to pressure from all of these forces.
