If you check the SEC filings on Opko Health's website you will see that Robert Baron exercised 100,000 stock options on 01/07/14 and subsequently sold stock to pay taxes from the exercise (what portion of the sales is for this purpose we'll never know). What's interesting is a number of Opko insiders, including Robert Baron, have recently exercised options early; some of those options had expiration dates more than a year out-2015.
