What exactly are the "SAE-reporting standard criteria", and where are they cited?

The clinical trial doctors repeated "Per standard criteria" a few time in one of their ASH presentations:

Both the FDA and Ariad should know what the standard criteria exactly are. The should follow the same standard the doctors follow. They shouldn't change the standard criteria at will.
