The Company maintains our principal office at 3400 SE Macy Rd., #18, Bentonville, Arkansas 72712. Our current office
space is leased with a base cost of $15,256 per month and runs
through September 2015 with options for additional lease
terms. The space consists of approximately 6,956 square
feet. We have an additional 1,800 square feet of adjacent
space at a cost of $2,100 per month for administrative and billi
ng services. The lease for this space runs through August
2015. Should we need additional space due to the continued gr
owth of the medical services, we do not anticipate any
difficulty securing alternative or additional space on term
s acceptable to us. During 2012, the Company purchased a
parcel of land in Tulsa, Oklahoma for approximately $792,00
0. The Company began constructing its second location on
the land in November 2012 with an expected completion date in October 2013. The land was purchased under our
construction loan for the Tulsa facility. See Note 6 under “Not
es to Consolidated Financial Statements” included in Item 3
in our filing on May 15, 2013 for the three months ended
March 31, 2013 for additional information on our construction
loan.
