My question is isn't the main risk here the cardio risk?



It is. But my bet is that the cardio risk decreases in earlier stage patients. In the FDA review, the ischemic events were related to age, time from first diagnosis, prior ischemic event, diabetes and hypertension.

Working with earlier stage patients lessens the impact of some of those risk factors.
