The obvious counter examples are:

a) Provenge - which had no SAE other than mild flu-like symptoms

Right. But I would venture that the efficacy of Provenge is mediated by an MOA that remains largely unknown / idiopathic. There are very small pieces of evidence that it elicits a response against PAP alone, which I find to be more anecdotal than scientifically convincing. But I'm deluded in this matter.
