After a quick read, looks like its much much easier to fill out the form 211 (a)3 <Filing> than to stumble through (a)5.
I mean, think of some of the stocks that need this to be done, they couldn't even get through the "Business/Products/Services" question....

PS - This means that there is actually someone that wants to buy & someone who wants to sell 400K shares, or only one "customer", and the MM acts as the other?
MM's can only quote unsolicited meaning they must have valid orders from bona fide customers that are not broker/dealers.
