As a Non US residence, I just called Epiq @ 150335977691

I told them I own ONLT CT's, and the reply was:
You need to fill W8 form & OFAC clarification and mail it to them ASAP.

Frankly, the lady I talked to didn't seem to know the difference between CT's, Preferred & Common shares.

I told her I don't have a claim no., she said they'll send me one!!
