I am talking about documenting the conversation, rather than documenting whether or not a telephone call occurred. People can make statements during a conversation and deny that they made that statement. To avoid such problems, I'll simply continue to insist on public statements in writing, such as PRs and SEC filings. Content even in these written statements is not always true; but if not true, they can be readily disputed, rather than dealing with "he said, she said."
