Would it be too much to ask for, if they supposedly consult in DTCC matters, that they avoid DTCC issues themselves?

Of course not.

Huge incompetency, no matter how it's excused.

Incompetency in one things tends to lead to incompetency in others. The fact that issuing "immediate" when in fact it's been anything but, is sign of other incompentencies, if not outright dishonesties, which also tend to occur more than just once.
