I think it is a "feed" issue. That is most of the brokers use the same feed service but all of them don't. So, my guess is that who ever was feeding from the system that made that transaction didn't get if fed to E*Trade and some others.

I don't think it happens often but I don't know that either as I only watch a few of the billions of transactions that take place.
