I am not sure how he could make it any clearer:

"Efficacy data for both cytokines and clinical endpoints will come from the subsequent 78 patients"

In another email to him I asked him specifically if 100 or 78 would be used for the secondaries, and he said 78. He has been pretty explicit about it.
