Most of those symbol changes are due to tardy filings. The problem is that Accounting firms dont have enough power, it seems that the clients wish to fight everything, if they feel it might damage their earnings and/or Balance sheet. So they fight it out with the accounting firm and end up either firing them or the accounting firm resigns. Then the company is tardy in filing and the stock ticker is changed, ppl sell like mad and the average investor who bought the company as a buy/hold loses on the steep and quick decline.
