For me there was no initial form I just had to get the DRS process started. I can explain my process with Zecco (hope it helps) in-order to have my shares DRS back to the TA I could not just call in to Zecco and request it. I had to type up a request and send it in to them. You can see what I sent them below. (With E-trade all you have to do is call them on the phone and they will start the DRS process to the TA)

I am requesting that 3,000,000 of my Signature Devices Inc (SDVI.PK) shares be DRS back to the transfer agent and held in book entry (electronic format) at the transfer agent. Please use the DRS Profile system the LPA # is 7900. If you are not able to use the DRS Profile please DWAC Deposit my shares with the transfer agent.

Once that was stared I went ahead and mailed in my stock power form with the medallion sig attached. I also sent in my request that my shares be transferred to Graffiti along with the fees associated with this process.( My guess from looking at their fee chart was that this was going to cost me $25 if it turns out to be more I'm sure they will let me know.)

I'm always nervous when giving out my SSN but its about of the process
