I'll assume you do not know the orgins of MTV.

It started on a special system only in some city in ohio not sure the name. It was basically an interactive group of stations that allowed users to have a direct feed back with the station. MTV was called something like sight & sound and showed concert footage and allowed viewers to vote on things here and there. Once the station got popular enough and the formula and shows were worked out over 4 or 5 years it was renamed MTV and started viewing in New York.

Now that's the timeline I am talking about. From idea to finalizing what would work. If you look at the timeline for MTV becoming national you can add another 3-4 years I believe.

So it started in 1977 and became national sometime in the mid-80's. The size and number of people having cable did not keep it from maturing quicker. In fact it probably made it easier since the market was not saturated already.
