Cracked vials, glass shards in vials... Why?

It seems several companies have had this problem lately.

Has quality control completely overlooked the vials? Are they coming from a careless manufacturer?

Or, especially with cracked vials, are the fill and finish machines not well programmed or calibrated, so cause damage?
