MINNEAPOLIS--(BUSINESS WIRE)-- Fairview Health Services and Accretive Health have signed a five-year agreement to bring a leading-edge care coordination solution to Fairview’s provider network and the patients cared for within the network.

Through the agreement, Fairview physicians and their care teams will have access to robust data, tools and processes to more fully understand the health need profiles of our patient populations, establish care plans that ensure the patient’s well-being across the care continuum, and track the status of those care plans on a real-time basis. Ultimately, the relationship is about better care coordination for patients resulting in the right care at the right time and lower total cost of care.

Accretive will provide people, processes, and technology that will be integrated into more than 40 Fairview primary care clinics and other Fairview network clinics. Clinicians will be supported with a comprehensive clinical care coordination solution, including necessary tools and expertise to be a medical home for patients. The care coordination capabilities also are key elements in the proposed Medicare Accountable Care Organization structure.
