I'm a broker in the process of creating a brokerage firm, the firm hasn't yet been cleared to do business but it has got through enough hurdles that it has a CRD#. Generally firms are first created from a mail address due to the time it takes to create the firm, as the firm approach's clearance date they move to an office, we have moved to an office as of 3 weeks ago.
