I was surprised as well considering RCC is increasing....over 200k new cases diagnosed world-wide last time I checked. Anyway, here's the EU cut-off from the PR:

Orphan medicinal product designation is awarded by the EMA for drugs that are shown to diagnose, prevent or treat life-threatening or rare, serious conditions that affect fewer than five in 10,000 people in the EU.
