ONCY - randomized trials

someone here had a beef with lack of randomized trials (lack of) for ONCY.
Randomized for sugar pills or standard of care? We know the baseline of standard of care and i have not heard of sugar pills having any effect on cancer patients.

Not sure how randomized trials would prove worthwhile. Please comment
