How GSI works. GSI has contacts in the medical industry who are medical practitioners of various persuasions. GSI buys receivables from those parties who wish to sell their receivables at a discounted price to GSI, as the practitioners need cash flow and want their money in a shorter time frame than that in which they will be able to collect it, also taking into account that the collection process is quite bureaucratic and cumbersome, and is much more effectively handled by a collections specialist than a medical practitioner, who would rather be seeing patients than trying to collect receivables. GSI is willing to wait, and contracts out the collecting process to a specialist who receives a fee from GSI and collects the receivables over an extended period of time, turning over the proceeds to GSI. GSI makes the spread, which is fairly substantial, between what the collector collects and remits to them less what they paid and the collection fee. As GSI's overhead is very low, a nice profit margin results. Nothing unique or magic, just a nice business.
