It's orders that have been placed previously, GTC orders, I can almost guarantee it.

It happens to me periodically, I'll have to be away for a couple weeks and can't keep in touch with everything so I place some GTC orders and come back to find they sold or didn't.

Of course, you can never discount the idiot factor.
