I imagined that to be the case. I work in this field as well and that is usually how it turns out to be. The employee was doing something that should have never been done. I am certain they were not trained to handle it the way it was being conducted. My guess is the employee was ready to go and took one of those short cuts that ended up costing them dearly.
this is the one thing that really frustrates me. An employee gets killed, while they are in violation of the safety program, and now the family wants to sue.
