This is a Process Control Problem. This is a thirty year old technical problem called " Glass on Pads".
Looks like we're both wrong, not native oxide, not glassed over pads. My bad, It looks like an improper Pad Etch didn't fully open the Pads. I still don't understand how they got by inspection though.
