He just filed for Appeal. However, he did not attach the $455 filing fee. The Clerk sent him a letter (to his old address) telling him to either (1) provide the $455 filing fee or (2) an Application to Proceed on Appeal in forma pauperis. The letter also stated to "Please complete the attached appeal information sheet as directed." Since it was sent to the only address the Court has on file for him, I assume he is not in possession of the "attached" information sheet.
Should he file in forma pauperis, proving to the Court that he is a pauper, and that Application is granted, he will be provided with an attorney. If he pays the $455 filing fee and the Application for Appeal is granted, I would assume he would have to have an attorney. I don't know how in the world he would maneuver an Appellate Court pro se.
