Partially true.

Who fills out the reverse split forms and what happens if the information filled in, is not missing, but incorrect for the split at hand?

As I stated before, I am not sure what the meltdown was with ICBS. It could be the above statement or lack of follow through information from the transfer agent or an employee at the DTCC that did not record the information correctly.

The only thing that was certain is that shareholders were not going to get shares if no delivery date is named while the transfer agent is waiting for shares and the DTCC is waiting for the name of a transfer agent to deliver shares to.

I have called corporate data of the Nasdaq several times asking about split information and it seemed that the number 1 answer for problems were incomplete and incorrect reverse split and other split forms from the company.
