The certificate, the letter demanding transfer, and the check for the fee must all be sent together. Call the transfer agent monday morning pertaining to this and ask to speak to Jason. Ask him all your questions and verify everything with him. He will tell you what to do. Do not do this by email, talk to him on the phone. Unless of course that is what you wish. Personally, I suggest the phone.
