It's not clear to me what the last page is. It could have been prepared by Kulwin to be adopted by the clerk, i.e., he filled the default form out for them, or it could be an official document from the clerk. It is hard to tell because there is no signature. It is also strange how some of the text on the default order looks like it was typed in on an editable .pdf and other parts on a typewriter.. Not enough info for me to make a conclusion... yet
