Ameritrade Refuses to Sign Form. Now What?
In response to a faxed request to sign the Share Certification Form (faxed to Ameritrade at 816-243-3769 ATT: Reorganization Dept) the following email was received. The cover letter did not request that any info be sent to any company, only to the sharehold.

Atrade Response:
Thank you for giving me the opportunity to assist you today. In regards to
filling out the form you faxed into us, because of the sensitive account
information requested, TD Ameritrade will not submit these forms since it would
be a violation of our privacy policy. The company may obtain a shareholder list
through standard means.
According to your account information with us, you are set up as a Non-Objecting
Beneficial Owner, meaning you do not object to having your name released as
being a holder of record or objecting to receive corporate communications.
