It has been the practice in all the cancer trials I've heard about for many years now that in trials that offer a new standalone medication, everyone gets the drug. This is for exactly the the ethical reasons you mention. We do have trials where there is one group that recieves a currently approved medication and another group that receives the current med plus the new one.
