He has to be fit to stand trial, or the trial is not fair.
But he needs to present medical evidence of genuine illness.
You could speculate that one purely hypothetical reason why his lawyer may have been withdrawn would be if he had become or otherwise been made aware of unethical / illegal conduct of his client in the pursuit of his defence e.g. falsifying ill health to delay the trial... but that would be putting 2+2 together to make 500.
If Berman's illness is genuine (and given that he is on trial for fraud and lying, you have to admit the possibility that it might not be!) , maybe that's karma catching up with him?
Whereas if he is malingering, to try and avoid having to face the consequences of his actions, I expect he will be caught and filleted. Malingering is a serious offence in its own right.
Another reason that his counsel may have withdrawn is because Berman was genuinely potless (as he claimed in the civil action that returned a default judgement against him) and unable to fulfill the terms of his representation agreement.
Then that leaves him both skint and in ill health. Which would be a justice of sorts.
We'll see what comes forth from teh hearings on 17th/27th Jan.
