I logged into the TDA platform (not the think or swim version) and went to the message center section. It'll show you a section to write a brief explanation of your issue and allow you to upload forms which is where I uploaded a photocopy of the Medallion stamped DWAC form. I got my confirmation response within the platform that my request was completed on 1/26. They'll probably get the original form eventually but that just helped me move things along I think. Good luck
