It's a constant struggle. Healthcare companies are clever and diligent billers, they bill for a wide variety of services through CPT codes, they may accept or not accept different insurances and even patients depending on perceived profitability. They are very good at pushing limits and billing as much as possible, while trying to help each patient. On the other side of the coin insurance companies and Medicare/Medicaid want to spend as little as possible and constantly make changes trying to reign in providers and keep costs down. All IMO, I work in healthcare but in a clinical role not billing, have seen injustices on both sides.
