I believe at one time it was reported that a handful of patients had been on no drugs at all (memantine or otherwise) in that first trial but the number was so small it was included with the memantine free (for 30 days at that time) which made up the 15 of 16 group where scores improved so well.

One thing that was dropped at that time was that many patients ON memantine improved as well (over half) but it was nothing like the 94% memantine free patients. Essentially if you were ON memantine you might be getting benefit, but if you were OFF benefit you universally did do better.

Another thing to think about is how much of the 30 or 60 day memantine issue is to do with the pathways memantine blocks or receptors that have been affected...or to patients who come off memantine after years of use, actually have a brief period of time that they actually do better creating a placebo response? I doubt that's ever been tested in any trial and if it lasted for few months, could also be a variable in the shorter term results from the last 2 trials.

One last thought is just because we are testing slightly less severe patients this go around, there are no guarantees that longer treatment such as this second cycle of treatment wouldn't be beneficial to even the most severe patient. We don't yet know and it really isn't as important as seeing who would be helped the most and at what stage.
