IMO, I think all of that requested info was tailored to the Latin America crowd for becoming a member of the "Fundacion" and the same form was used to accept shareholder account creation requests for the PDR dividend. I noticed that the first rev of the form had some words in Spanish, which leads me to believe this. All of the info they are requesting would be perfectly reasonable if you were creating an account to purchase PDRs. People have to try to keep in mind that the company isn't a debit card company anymore. The company Panamersa is the holding company for a number of Latin American companies, one of which is Fundacion Paypro. While the forms Fundacion Paypro uses in Latin America may seem a bit odd to us gringos, I really do not think there was any attempt to collect unnecessary info in the form. Like people have said before, put NA where you think it is appropriate for the time being until Fundacion Paypro edits the form.
