I also have some shares in my etrade account as well.
And they finally sent this to me.

Terms: Exchange at a rate of 1 share of the New Series C Preferred Class for every
250 shares of common.
Please Note: New shares will be restricted for 2 years and held in book entry
@ the Agent. Holders will be charged a $160.00 fee per instruction submitted by the Agent.
Should you decide that you prefer a stock certificate instead of holding the
shares in book entry form, an additional $85 will be required for the cost of
the certificate and shipping. Additionally, there is a $38 reorganization fee
charged by E*TRADE for these requests.
Holders must complete the "Tender Share Conversion" Form and submit to
E*TRADE in order to participate in this exchange. If you do not wish to participate
